Comparison of PRP treatment and corticosteroid injection for frozen shoulder and peri-arthritis.

Updated: Feb 27

Comparison of the Efficacy of Platelet-Rich Plasma (PRP) and Local Corticosteroid Injection in Periarthritis Shoulder: A Prospective, Randomized, Open, Blinded End-Point (PROBE) Study (Oct 2022).

Comparison of PRP treatment and corticosteroid injection for frozen shoulder and peri-arthritis.

Periarthritis and frozen shoulder (adhesive capsulitis), is characterised by Progressive and long-lasting stiffness and pain along, more common in the age group of 40-60 years. The various treatment modalities used for its treatment and management include physiotherapy, manual therapy (osteopathy),ultrasonic therapy, TENS, hydrotherapy, pain medication, intra-articular injections, manipulation under general anesthesia, and surgical management.

Joint steroid injection has been a common and effective treatment option in deducing shoulder pain. Some recent studies reported a better outcome using platelet-rich plasma (PRP) injections in theses cases, with better long-term outcomes. This randomised controlled trial was conducted to compare the efficacy of PRP injection treatment and steroid (cortisone) in patients of shoulder periarthritis.

This study concluded that both intra-articular injections of PRP and steroid were effective for treatment of frozen shoulder/periarthritis pain and disability.

Patients were monitored within the fourth week, 6th week, 12th week, 24th week and beyond. Like with many studies, the steroid group tends to be a little bit more affective in the short term. However, this tends to be short lived. Whereas the PRP showed better results in long-term outcomes (up to the 24th week and beyond). Future studies that compares single versus multiple injections of PRP injections would be good to further analyse progress and pain relief. Again, PRP is reported to be the safer option, with less damaging effects to the joint cartilage.
